Hello, I'm so sorry you are going through this with Scarlet. I have reviewed the previous exchanges you had with our veterinarians and I do have to agree with both of them and state that seizures as well as respiratory distress, which I can see that you also asked about, are not typical signs of amikacin toxicity. Definitely, deafness and vestibular signs can occur when an aminoglycoside antibiotic such as amikacin is allowed to enter the middle ear, which commonly happens when the tympanic membrane is ruptured. This is why it is so very important to ensure that the eardrum is intact before prescribing and using these preparations. Sometimes the TM cannot be visualized due to the high degree of swelling present in the canal or the amount of debris accompanying the infection, and when the drug is used in these cases this is often how the drug enters the middle ear and does damage. Unfortunately, sometimes we have very little choice in the way of antibiotics we can use to treat serious multi-drug resistant ear infections, such as those caused by bacteria of the pseudomonas species, which is when amikacin is typically used. If the seizures were in fact due to the amikacin, that would be a highly abnormal reaction to the drug as it was prescribed in Scarlet. It would be very important to me, were I her doctor, to rule out other diseases that cause seizures, specifically brain lesions like tumors, before concluding that this was indeed a drug reaction. If no other issues are found that could explain the seizures, I would urge your veterinarian to contact the manufacturer of the preparation and report this as an adverse effect.

Unless the eye infection is caused by something else in the body, systemic antibiotics are rarely used or indicated. Ophthalmic antibiotics are much more likely to cure an infection of the eye in a timely manner. Sometimes what looks like an infection isn't actually caused by bacteria. It could also be caused by corneal abrasion or ulceration, allergies, foreign body, autoimmune disorder, etc. I also prefer other types of injectable antibiotics over gentamicin. It is an aminoglycoside antibiotic that can be kidney toxic and has a pretty long list of adverse effects. I only use it in cases that do not respond to safer antibiotics.

Unfortunately this is going to be quite the difficult case to manage due to such the resistance to antibiotics. In this case it would be worth trying a multi-antibiotic approach and you may need to resort to an antibiotic such as Amikacin, which can have adverse effects on the kidneys. Unfortunately, I see that Amikacin is also on the list of antibiotics that this is resistant to. There are more alternative methods of treatment such as ozone therapy in the bladder which may help kill the bacteria. I would discuss this further with your veterinarian as treatment will depend entirely on what the vet wants to use. Scotch may need to be admitted into the hospital for treatment for IV antibiotics as well. I hope this helps guide you in the right direction. Gentamicin can be given by the intramuscular, subcutaneous or intravenous routes. It also comes in topical preparations for eyes, skin and ears. You should only give this drug under guidance from a veterinarian- it can cause serious side effects including kidney failure.

If you think Cotton Ball is in need of an antibiotic for a infection, I would recommend having him examined by a veterinarian so that the most appropriate antibiotic can be recommended. Antibiotics such as Convenia and Doxycycline are good choices for antibiotics when treating respiratory infections. I hope this helps guide you in the right direction. Using antibiotics is generally safe for pets with kidney failure, however Sofiya needs some aggressive treatment for her kidney issues. This could look like more testing, including an abdominal ultrasound, leptospirosis testing, checking for tick diseases, and other tests depending on what is going on with her clinically. It would also include treatment of IV fluids, anti-nausea medications, medications to lower her phosphorous levels, and others (again depending on how she's actually doing). In short, the Cefor isn't going to treat the issues in this bloodwork.
