My senior dog has severe kidney and liver damage. What can I do?

I am so sorry to hear about Lola. It is so hard with senior pets to know when it's time to say goodbye. I know how hard it is as I lost my 15 and a half year old guy last year. The blood work values you report are very concerning. The question is if there is something that has acutely changed to make her become a lot worse, or is this simply progression of underlying chronic kidney disease. There are other disease processes that can complicate chronic kidney disease like infection, pancreatitis, high bood pressure, infectious diseases, etc. Really what your pet needs is to be hospitalized with intravenous fluids, supportive care (anti nausea medications, etc.), and to try to figure out if there's an underlying problem exacerbating the kidney disease. This will require more tests such as an abdominal ultrasound, urine culture, and potentially other things as dictated by the case. I would not give up hope until trying these things because you won't know if this will help her until you try. The opposite would be true if a pet in this situation were hospitalized for two to three days and there was no improvement in the blood work or symptoms. Then, if all the treatment options are exhausted, and the pet has poor quality of life a different decision may be reached. As dogs do not tolerate chronic kidney disease as well as cats being diagnosed for a year without progression is excellent. Sometimes having a consultation with an internal medicine specialist can really help. Your family vet would likely be able to provide a referral if you and he or she thinks this would be a valuable step. I hope this helps and that Lola feels better soon. Poor Bruno! I can see the brown material in his vomit and the very dark stool, which is likely digested blood. This happens because acids build up in the blood when they are not filtered by the kidneys, these acids are very caustic to the tissues of the stomach, and result in ulcers, which bleed. The prognosis depends on the cause of the kidney disease, as certain conditions can be treated and managed for some time, but unfortunately, the prognosis is typically poor to grave. Bruno likely needs fluids to prevent dehydration, and medication to protect the lining of his intestines. The kidneys are also responsible for making a hormone, erythropoeitin, which helps the bone marrow make new red blood cells. Along with the gastric bleeding, this is causing the drop in hemoglobin. The decision is not easy, but humane euthanasia may be an option to consider for Bruno, as kidney failure is known to be painful and very uncomfortable in humans. Unfortunately, treatments such as subcutaneous fluids and toxin binding agents such as Azodyl and Epakatin are palliative at best. I am happy to answer any additional questions you may have.

The most helpful treatment Lio could receive right now would be IV fluids for hydration. This will also help diurese him to flush out any potential toxins from his blood. With his age I would be concerned that the cause is a toxicity from him eating something possibly. Denamarin is a good liver supplemw t and getting him to eat a critical care formula food such as Hills A/D is good. If an abdominal ultrasound has not yet been performed, I would consider having that done for more information. I hope this helps and that Lio improves soon. It looks like he is suffering from some liver and kidney values elevations and he also is anemic. (Low red blood cells). Since I do not not his current condition and how long this has been going on, it is difficult for me to give you a survival time. If this has happened quickly and could be a toxin then with aggressive therapy (hospitalization and IV fluids), then these values could go back to normal and stay that way as long as there wasn't any permanent damage. If this has been chronic and his urine is very dilute then this could be permanent damage to the kidneys which is a much poorer prognosis long term. I would still recommend hospitalization and IV fluids if possible. I would talk to your vet about whether he needs fluids, antibiotics, appetite stimulants, anti-nausea medicine etc. I hope this helps and your sweet baby gets well really soon!

I'm sorry to hear that Caramel is not feeling well. The paperwork showing "female" instead of male is a typo somewhere. Nothing that was run in those tests determine's the dog's sex, so I suspect that was entered wrong at the lab. His kidney values are a bit high, but they aren't crazy high. If a urinalysis was not performed, talk to your vet about running a urinalysis as well, as this will help determine whether those values being high may be in part due to whether or not he is dehydrated (typically the "USG" or "urine specific gravity" is what we are looking for to help interpret this). You may also want to talk to your vet about an abdominal ultrasound (sonogram of the belly); having images of the kidneys and bladder as well as the rest of the abdominal organs (liver, pancreas, GI tract, spleen, adrenal glands) can be helpful in terms of what the most likely cause of his clinical signs are. Another consideration since he is "hardly able to get up" would be a painful condition that's making him not want to move; ask your vet if he seems painful anywhere. To summarize, here is what I recommend you do. Take him to your vet and ask about a urinalysis. Ask your vet if he seems painful anywhere (belly, back, neck, joints). A urinalysis would be needed to see if the increased kidney values are because of kidney disease or dehydration. I am concerned about liver failure causing the low protein levels and hypoglycemia. Additional diagnostics would be needed to determine the cause and most appropriate treatment. An ultrasound, biopsy or culture may be indicated. Symptoms can be treated individually, so discuss the use of anti-nausea medications with your vet. Other medications may also be indicated to reduce the effects of digestion with a failing liver, like metronidazole, lactulose, denamarin, and others.

I'm sorry your dog isn't feeling well. Regarding your comment about leaving kibble in his bowl without eating it, yet still eating 2 cans of food a day and some treats--it sounds like he is getting enough food and appetite is not a problem. When you say he is "pooping everything out," is he having diarrhea? Or does it seem that he is losing weight even though he is eating a lot? Or is he pooping very frequently? To review the list of problems/symptoms that you have given: *losing weight even though he is eating a lot (if this is what you mean by "pooping everything out") *uncomfortable/moaning *enlarged liver *blood test negative for Cushings disease Considering that he is 13, given the problem list above one of the biggest things I would worry about is cancer of some kind, and arthritis may be contributing to his discomfort. Unfortunately there is not any ONE test for cancer, so its not something your vet can just submit a blood sample and get an answer for. If you are wanting to KNOW what her diagnosis is, your vet will need to run full lab work (a full chemistry profile, a complete blood count, and a urinalysis), take X-rays of his chest, and probably perform an abdominal ultrasound (sonogram of the belly) as well. If an abnormality is discovered on X-rays or ultrasound, a biopsy may needed. Regardless of the cause, and even if you don't have funds to do a lot of diagnostics, you can focus on making him comfortable, with the understanding that his time may be limited. Ask your vet about pain medications. Don't worry about him not eating kibble--if he prefers canned food that is fine. I'm sorry I cannot give you black and white answers as to what is wrong.
