Why does my older terrier have a dent in her head? Is it serious?

I am sorry to hear about Pepper! A swollen face can be caused by a variety of issues, including an allergic reaction, a bite from an insect, or an abscessed tooth to name a few. These are all treated with different medications, so it is recommended to bring Pepper in to the vet to examine her to determine what is causing the swelling and how best to treat her. If the swelling is increasing in size or she is struggling to breath it is a medical emergency and she should be seen immediately.

Poor Pepper. She may have injured her eye or could have an eye infection. Any eye problem needs to be checked out by a vet promptly, as leaving it untreated can lead to vision loss. I'd take her into a vet now.

It's really difficult to say what this is without seeing her and being able to evaluate her. It may be a normal structure, or something else like a defect, or cyst. I would need to have more information and see it to be able to have a good idea what you are talking about. There could be a few things that look like that. An infection that ruptured through the skin, a bite wound, a fly larva or a cyst that popped could all have this appearance. I would recommend to take Pepper into the vet within the next 24 hours to have an exam and likely antibiotics. She really does need some investigation into what is going on. If toxins can be ruled out and there hasn't been a traumatic injury to the head it is most likely going to be a central nervous system lesion. Unfortunately this is normally a brain tumour or similar. Epilepsy is still a possibility but rarely occurs in older dogs. I would recommend seeing your vet as soon as possible so they can start with some blood tests and take their investigations from there. Fingers crossed for you.

The reason you are noticing that the top of his head is more prominent is because of muscle wasting of the head muscles. This is a common in older dogs and often times it can be due to cancer. The "cone" or pointiness of the head is the occipital bone. I suggest you get Sammy to your vet for an exam. They can give you better advice since they can get a hands-on examination.
