My Chihuahua has abdominal pain. Pancreatitis or kidney problem?

I'm sorry to hear about Chi Chi's recent issues. Vomiting and an anal gland abscess/rupture may cause some changes on the blood panel, but not typically ALP. When you have a significantly elevated ALP and normal ALT it is highly suggestive of Cushing's disease. Cushing's can be diagnosed with a few different tests, these tests involve taking blood, injecting a substance then taking blood again to see the body's ability to respond. I think it would be wise to have her checked. If she dies have Cushing's thankfully it is treatable with medication but this will be for life.

Symptoms like this can be related to pain in the back or pain in the pancreas. Even though she is acting normal, the tense areas indicate there is some discomfort and I recommend to have her examined by her veterinarian. The doctor may recommend radiographs to check the spine or a blood test to check the pancreas. Both conditions should be treated to alleviate pain. In looking at the bloodwork you attached, I would not be concerned about the total White Blood Cell value being slightly lower than normal. After the total WBC value, the bloodwork breaks it down into individual white blood cells, like neutrophils and lymphocytes, none of which are too low. In fact, neutrophils, the body's first responders to infection or inflammation, are a little higher than the normal range. White blood cells are the fighters of the immune system, and it appears that Mary still has plenty in case she needs them. There is nothing you can do raise them other than keep her in good health. However, I do see that both BUN and Creatinine are also elevated. This can happen for many reasons, including severe dehydration, kidney insufficiency or failure, kidney infection, among others. These are the more concerning numbers that I see, and I am sure your vet has discussed a plan of action for these values.

The elevated wbc count is common with both inflammation and infection of damaged tissues and although should be monitored is not abnormal given recent pancreatitis. An ultra low fat but not calorie restricted diet is generally considered best for recovery, liquid recuperation supplements are also very helpful as the gut recovers

BUN and CREA are severely elevated and the cause of it should be ideally understood by performing a urine analysis (and specifically a urine specific gravity to rule out kidney failure). Mild anemia (lower RBCs) can be present during kidney failure. Amylase is another important value as severe elevation could be compatible with pancreatitis, which can cause the signs you are describing. CPLI test can be performed as well to rule out pancreatitis. Please discuss all the options with your local vet.

Poor Abby. I am concerned that she is having a different problem than the pancreatitis. My biggest concern is that she may have a inflammed disc in her neck that is cause her a lot of pain and/or some neurological issues. I would have your vet do an exam and see if she needs some anti-inflammatories and pain meds. Your vet will be able to do some spinal X-rays if needed. Based on your post of Lucy's bloodwork she has both elevated liver and kidney values which could mean she has another underlying condition altogether that is affecting both her organs such as leptospirosis, Cushings disease (hyperadrenocorticism), or any number of tick diseases. Therefore she needs additional testing to determine what the primary disease process is. She needs to most likely be hospitalized, placed on fluids to flush her system and pain medication if indicated. while the vet runs the necessary tests to determine the underlying problem here so that we know how to best treat her. Like I said before these lab values cannot be differentiated unless additional testing is done. I would likely first start with finding the specific gravity of her urine to rule out kidney failure first. Just because elevated kidney enzymes are present it does not always automatically mean that the pet is in kidney failure. Also it is very unlikely that both the kidneys and liver enzymes will be elevated without having another underlying disease process causing this.
